Point release with fixes

Hey all,
This is a point release of Smallest Quest, fixing a number of bugs. Most of these were found by Mark Alan - thanks Mark!
- Fixed a bug when the witch character dies.
- Added more buttons for controller support.
- Fix bugs with item creation.
- Pressing enter/button dismisses messages.
- Fix gamepad issues on Mac & Windows.
